Origyn – Twin IVF babies after 8 IVF Failures including 4 miscarriages

These babies (twins) , Origyn babies were born in the very first successful attempt at Origyn IVF one of our case study of March 2017.

Mrs. Vandana (name changed ) 43 Years female came to us with previous one ectopic pregnancy 8 IVF failures : 4 Self egg & 4 donor egg and along with 4 miscarriages (all IVF pregnancies). She had lost all hopes of delivering a baby on her own and came to us seeking surrogacy. After screening all her reports, she was told to undergo tests for so many miscarriages and then proceed with whatever treatment is decided by them in consultation with Dr. Rashmi Sharma. The patient had poor ovarian reserve also her AMH was < 1.0 along with male factor (Oligoasthenozoospermia).

These steps were taken to improve pregnancy outcome as the patient had experienced many pregnancy losses before.

  1. Endometrial scratching: the endometrium is gently scratched with a cannula to make it more receptive to embryo implantation. It is an OPD procedure.
  2. Embryo Glue : In case of previous IVF failures a new culture media is giving promising results.
  3. Heparin: An anticoagulant used in cases of thromboptic disorders.

After all these measures Vandana delivered a baby boy and baby girl little pre-mature at 32 weeks through C- section in March 2017. Now she is a proud mother of 2 babies.

Recurrent Pregnancy Loss (RPL)

“No matter how much time passes there will always be this hollow place deep inside that throbs for the one who is gone.”

Pregnancy loss occurs in 10 to 15% of all pregnancies, of which 1-2% are recurrent. The World Health Organization (WHO) has defined miscarriage as the loss of a fetus weighing <500g, which would normally be at 20-22 complete weeks of gestation.

Recurrent abortion is traditionally defined as three or more consecutive miscarriages occurring before 20 weeks.

ASRM guidelines suggest two or more clinical miscarriages (Ultrasound Documented) but may not be consecutive.

When to start testing?

(Do we wait for 3 abortions before start testing?)

  1. No at 2 abortions we can start testing the causes of recurrent pregnancy loss
  2. In some cases even at 1 abortion testing is done like:
    1. If women is older >35 years
    2. Pregnancy after infertility treatment.

Causes of Recurrent Pregnancy Loss:

Genetic causes

  • 2-4%
  • Associated with parental chromosomal abnormality in which most common cause is balanced translocation.

Infectious Factor

  • 0.5%
  • Toxoplasma gondii, rubella, HSV, measles
  • Direct infection to the uterus, foetus or placenta

Immunological Factor

  • 3-5%
  • Antiphospolipid syndrome

Anatomic factor

  • 10-15%
  • Endometrial factor
  • Congenital uterine anomalies
  • Intrauterine adhesions
  • Uterine fibroids or Polyp
  • Other mullerian anomalies like: unicornuate, didelphic, bicornuate
  • Asherman syndrome

Endocrine factor

  • 17-20%
  • luteal phase defect
  • PCOS
  • Diabeties
  • Thyroid disease
  • hyper prolactinemia

Environmental Factor

  • Exposures to
    1. Smoking
    2. Alcohol
    3. Caffeine

Diagnosis of Recurrent Pregnancy loss

Diagnosis evaluation mainly includes:

Karyotypes of POC/ parents

Uterine Anatomy (evaluation by 3D USG/ Hysteroscopy)

Evaluation of Thyroid Dysfunction

Testing For Diabetes

Antiphospolipid Syndrome

Selected Thrombophilias


Some of the treatments used in recurrent pregnancy losses are

1. Antiphospolipid Syndrome thrombophilia. Ecosprin & Heparin


2. Parental Chromosomal abnormality: Treatment may be either use of donor gametes or IVF with PGD

3. Unexplained recurrent pregnancy loss: No proven treatment yet available for unexplained reasons but lot of clinical trials are going on for immune therapy / G-CSF Therapy etc.

4. Progesterone Support

These are few slogans heard these days with awakening launch of nationwide campaign of “Save Girl Child” by our Hon’ble Prime Minister Shri Narender Modi ji. This has opened the eyes of society with the true facts of dropping ratio of boys versus girls i.e 10 : 7 which is an alarming signal for Indian social setup . Many celebrities and learned people have joined hands for it to make this campaign a success. Madhuri Dixit is the brand ambassdor for this project. Below is the story of my own childhood memories , which is itself an example of that girls are not curse and no less than boys.
Now being a IVF specialist , if I recall back at my own personal story and my childhood memories I feel on the top of the world to have been blessed with good parents . I have been brought up in a middle-class family setup, from a very small town of U.P , Khekra, but very lucky to have such a open minded and educated parents for whom we four sisters were not a burden.They made education our weapon and we all succeeded in our career. We were provided with all oppurtunities that are given to boys. I was always inclined to study in science stream but in our girls school there was no science , so I was admitted in boys school which was a revolutionary step itself in small town , khekra , and soon other girls of our town joined me in that school . I completed my higher secondary with a place in UP Board’s merit list than got into BHU for my MBBS and after that did all my higher studies further. My other sisters too are doing best in their career my parents have proven in Khekra that ,if we are dertermined no one can stop us to achieve the goal. Today my parents are very proud of their 4 daughters and say that they are the happiest persons in the world.My intention to convey that we should give equal oppurtunities to girl child and girls make parents much happier than any boy will do.
